About the Study
Bonafide Health invites you to participate in a research study designed to evaluate the effects of Lacticaseibacillus rhamnosus CA15 probiotic on vaginal health and overall quality of life. This is a 30 day trial. Individuals will be asked to take the probiotic daily, complete daily diaries and weekly questionnaires.
The purpose of this questionnaire is to determine your initial eligibility based on inclusion criteria for the trial. (Of Note: Completing this form does not guarantee enrollment or eligibility in the trial)
